A Department of Transportation (DOT) physical is a checkup that is required for all commercial vehicle drivers by the Federal Motor Carrier Safety Administration. This exam ensures you can meet the physcial requirements of your job.
What do you need to bring to a DOT physical?
You are required to fill out a medical examination report form. You can access the form here. Please complete the form and bring it with you for your appointment.
Other items you need for your DOT physical:
• Current list of all prescription medications, injectables, vitamins, and supplements. Include dosing and timing.
• Medical records from any specialists you see, particularly if you’re being treated for any sleep or seizure conditions.
• Recent lab results, if available.
• Prescription glasses or hearing aids if you need them while driving. You will be asked to use these during testing.
What does a DOT physical consist of?
You’ll be asked to provide detailed information about your health, including:
• Medications you take
• Surgeries you’ve had
• Alcohol, tobacco, and other substance use
• Health symptoms or medical conditions you currently have
At your DOT physical, you’ll be given a complete physical examination, which tests your:
• Hearing and Vision Testing
• Blood Pressure Check
• Tests for other health indicators
• Urinalysis
